Release of Liability:

Your safety is our primary concern during the dance whether it is indoor or outdoor.

I recognize and understand the risks of physical injury inherent to dance, and I fully assume all the risks associated with the event. I hereby release Tracy Reach from all liability for injuries sustained or illnesses contracted while attending or participating in any dance events/performances, including food and water consumption.  I agree to waive the right to pursue any claim, indemnify, defend, and hold harmless officers, volunteers, contractors & temporary workers of Tracy Reach, event sponsors, other vendor employees for liabilities, costs and judgments involving injury or damage to any person or property.

All the event participants must complete this form before making the registration. If the participant  is under age 18, a parent or guardian must accompany. 

Release of Photography and Video:

This event is photographed & video graphed. Attendance at this event represents your permission for the host to use the Videos & Photos.

Acknowledgement of release waiver:
In signing this release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless all the parties mentioned above. I am in agreement by reading it before I sign it. I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration, fully intending to be bound by the same.
